Lianne Chelman, MSW, LCSW. Lianne was born and raised in Franklin, MA. She is a Licensed Clinical Social Worker (LCSW). She graduated from Salem State College in 2005 with her bachelor’s degree in social work. After her bachelor’s work, Lianne worked as a social worker for a private not for profit. Lianne returned to graduate school and graduated in 2008 from Bridgewater State College with her masters in social work. She obtained her clinical licensure the following year. Lianne was a student intern for Jewish Family Service Metrowest during her graduate program. It was at this time she gained an appreciation for the aging. Her specialty is in geriatric social work, specifically geriatric care management. Having been trained by JFS/MW GCM, Lianne is skilled working with elders in the Metrowest community. At JFS/MW, she specializes in working with seniors and their families in the Metrowest community. Lianne is a member of the National Association of Social Workers (NASW). She is passionate about her work as a social worker and brings a kindness and consideration to her clients and their families. Patricia B. Herzog, RN, BSN, joins the JFS/MW Elder Care Team as its Clinical Supervisor. After graduating with a Bachelor of Science in Nursing from Middle Tennessee State University in 1991, Ms. Herzog began her nursing career in critical care and cardiology at Vanderbilt University Medical Center in Nashville. Over her 18 years of professional nursing, Trish has a range of experience with clients both in hospitals and within the community. Her career focus is now in Geriatric Care Management with an emphasis on the care of elders in the Metrowest community. As a member of JFS/MW’s Geriatric Care Management team, Trish assesses and coordinates the needs of clients, families and loved ones. Her primary goal is for her clients to remain as independent in the community as possible while continually assessing the changing needs of elders and coordinating the services necessary to meet those needs. This is achieved by an interdisciplinary approach within the agency as well as with providers within the Metrowest community. Trish is very excited to answer your questions and help caregivers and elders navigate through the many services available.
