The Patient Navigator Program helps engage older adults and maintain their independence as they age.

JFS of Metrowest is thrilled to announce that, thanks to the support of our many donors and volunteers, this week the program completed its 3,000th appointment!

Piloted in 2012, demand for the program has grown steadily with 386 older adults enrolled in the Patient Navigator program.

Patient Navigators are trained volunteers that accompany adults aged 60+ to their medical appointments, preparing them for their visit, taking notes during the consultation, making sure that follow-up appointments are made, picking up prescriptions and returning them safely home, reducing the stress often associated with medical appointments.

Volunteers are vital to the success of the program and enable JFS to offer this important service at no cost to the client. The demand for Patient Navigators is increasing with an average of 15-20 appointment requests each week!
