JFS recently welcomed Mindy Hogan, Account Executive with Fallon Health’s Summit ElderCare PACE Program, who met with our team to provide an overview of the services and supports available through the program for eligible older adults.

We appreciate Mindy taking the time to share information about Summit ElderCare PACE and answer questions about the program, helping our staff better understand another resource that may benefit older adults and their families in our community.

During the presentation, Mindy also shared information about Fallon Health, a not-for-profit healthcare organization founded in 1977. Fallon Health offers a variety of health plans and care programs, including Summit ElderCare PACE, and works with healthcare providers and community organizations throughout Massachusetts.

What is Summit ElderCare PACE?

Summit ElderCare is Fallon Health’s PACE (Program of All-Inclusive Care for the Elderly), which is designed to help eligible older adults remain safely in their homes and communities while receiving coordinated healthcare and supportive services. Participants receive care through an interdisciplinary team of healthcare professionals who develop an individualized care plan based on each person’s medical, social, and supportive care needs.

Program participants may have access to a variety of services, including:

  • Comprehensive medical care
  • Adult day health programs
  • In-home services
  • Social work & caregiver support
  • Care coordination
  • Transportation to medical appointments and PACE center activities

The program is available to eligible older adults throughout much of Massachusetts. Translation & interpretation services are available for increased accessibility and their team has experience working with Veterans and their families.

Understanding the Cost

Mindy explained that Summit ElderCare functions as a health insurance plan that covers the care and services participants receive through the program. Covered services do not require copayments, deductibles, or coinsurance. Depending on eligibility, monthly premiums may be covered through Medicare and/or MassHealth (Medicaid), and a private-pay option is also available. She also noted that many participants qualify for financial assistance, which may significantly reduce or eliminate out-of-pocket costs.
