• CRIMINAL OFFENDER RECORD INFORMATION (CORI) ACKNOWLEDGEMENT FORM
• MOTOR VEHICLE RECORD CHECK AUTHORIZATION RELEASE

To complete the forms electronically, please:

• Click the links to download the forms*
• Complete both forms in their entirety
• Save both documents with your changes
• Upload the electronic files through this link: JFS Dropbox account

* Note, you do not need to log into an Adobe account to complete the forms but your system will need a current version of the Adobe Reader.
To download the forms, look for the download icon at the top right of the window:

To complete the forms by hand:

• Click the links to download the forms
• Print them out and complete both forms in their entirety
• Scan all sides of both forms to an electronic file and upload through this link: JFS Dropbox account
