Job Title: Caseworker
Primary Assignment: Older Adult Services
Salary Range: Commensurate with Level of Education, Experience and Licensure (if applicable)
Work Schedule: 40 hour-Full time Position (Salary Exempt) with Benefits
Position Description:
Established in 1979, JFS is a trusted nonprofit supporting older adults, immigrants, and other vulnerable community members. We provide culturally responsive, client-centered services that promote safety, dignity, and independence.
We are seeking a compassionate, flexible, highly organized Older Adult Services Caseworker to support several older‐adult service lines, including Patient Navigator, Naturally Occurring Residential Communities (NORC) supportive services, and 60+ Case Management. This role is ideal for someone who values relationship-based work, teamwork, and problem-solving in a fast-paced environment.
Key Responsibilities
Client Support & Case Management:
- Complete person-centered intakes, assessments, and ongoing case management.
- Support clients navigating medical appointments, healthcare systems, and community resources.
- Provide health/wellness services and group programming in NORC settings.
- Maintain accurate, timely documentation and track necessary data.
Program Delivery & Collaboration:
- Collaborate closely with internal teams, community partners, and volunteers.
- Coordinate the needs of clients with the assignment of volunteers for medical appointment navigation.
- Supervise and train volunteers.
- Participate in interdisciplinary discussions and care planning.
- Support outreach, quality improvement, and data/reporting needs.
- Represent JFS with professionalism and compassion.

Qualifications
Required:
- Master’s degree in social work, gerontology, psychology, human services—or Bachelor’s degree with relevant experience.
- 1–3 years working with older adults, care management, or community health preferred.
- Strong interpersonal, communication, and organizational skills.
- Ability to work respectfully with diverse populations.
- Valid driver’s license and local travel availability.
Preferred:
- Multilingual (Portuguese, Spanish, Russian, Haitian Creole, etc.).
- Experience with home care, medical navigation, volunteers, or older adult services.
- Familiarity with Medicare/Medicaid, SNAP, housing, or Long Term Services & Supports (LTSS)
Work Environment
- Hybrid schedule (office, field, remote-eligible after 2 month onboarding/training period).
- Home visits and community meetings required.
- Occasional evening/weekend shifts to meet client, program and/or agency needs.
