Volunteer Forms and Links
Thank you for joining the JFS team of volunteers!
Here are two documents that all JFS volunteers are required to complete:
- CRIMINAL OFFENDER RECORD INFORMATION (CORI) ACKNOWLEDGEMENT FORM
- MOTOR VEHICLE RECORD CHECK AUTHORIZATION RELEASE
To complete the forms electronically, please:
- Click the links to download the forms*
- Complete both forms in their entirety
- Save both documents with your changes
- Upload the electronic files through this link: JFS Dropbox account
* Note, you do not need to log into an Adobe account to complete the forms but your system will need a current version of the Adobe Reader.
To download the forms, look for the download icon at the top right of the window:
To complete the forms by hand:
- Click the links to download the forms
- Print them out and complete both forms in their entirety
- Scan all sides of both forms to an electronic file and upload through this link: JFS Dropbox account
- OR mail both forms to JFS of Metrowest, 475 Franklin Street, Suite 101, Framingham, MA 01702, ATTN: Mandy Fleming
